Sufficient Certification Clause Samples
The Sufficient Certification clause establishes the requirement for a party to provide adequate proof or documentation that certain conditions or standards have been met. In practice, this may involve submitting certificates, reports, or other forms of verification to demonstrate compliance with contractual obligations, such as safety standards or regulatory requirements. The core function of this clause is to ensure that all parties have confidence in the fulfillment of key requirements, thereby reducing the risk of disputes and ensuring transparency in the performance of the contract.
Sufficient Certification. (1) Certification shall be sufficient if it states in writing:
(a) the date on which the leave will be taken;
(b) the duration of the leave;
(c) the purpose for requesting the leave;
(d) the employee's signature.
(2) The following certification form may be used by employers: I certify that on I will/did take hours of leave for the following purpose: Employee's Signature: Date:
Sufficient Certification. Certification provided above shall be sufficient if it states:
(a) The date on which the serious health condition commenced;
(b) The probable duration of the condition;
(c) The appropriate medical facts within the knowledge of the health-care provider regarding the condition;
(d) A statement that the eligible employee is needed to care for the son, daughter, spouse, or parent and an estimate of the amount of time that such employee is needed to care for the son, daughter, spouse, or parent; or, a statement that the employee is unable to perform the functions of the position of the employee, asappropriate.
(e) In the case of certification for intermittent leave, or leave on a reduced-leave schedule for planned medical treatment, the dates on which such treatment is expected to be given and the duration of such treatment;
(f) In the case of certification for intermittent leave, or leave on a reduced-leave schedule, a statement of the medical necessity for the intermittent leave or leave on a reduced-leave schedule and the expected duration of the intermittent leave or reduced-leave schedule.
Sufficient Certification. Certification provided under Section (a) above shall be sufficient when approved by the Human Resources office. The Employee shall be provided a reasonable opportunity to correct any deficiencies in the certification.
