Statistical Reporting. At the commencement of performance, Consultant shall complete and submit, and require each Subconsultant who performs services under this Agreement to complete and submit, a Self-Certification on the form contained in the Exhibits. At the completion of work and prior to final payment, Consultant shall complete and submit a Final Distribution of Contract Dollars under this Agreement on the form contained in the Exhibits.
Statistical Reporting. 17.1.1 At the commencement of performance, Design Professional shall complete and submit, and require each subconsultant who performs services under this Agreement to complete and submit, a certification in the form of the Self-Certification. At the completion of work, Design Professional shall complete and submit a report of the distribution of compensation received under this Agreement in the form of the Final Distribution of Contract Dollars.
Statistical Reporting. IMS shall maintain Customer's data within IMS' policy, claims and general ledger systems. IMS shall prepare and submit to FIA, monthly financial and statistical reports, reconciliation reports, certifications, and statistical tapes on Customer's behalf, in accordance with WYO Flood Program Accounting Procedures and the Transaction Record Reporting and Processing Plan ("TRRP Plan").
Statistical Reporting. Service Operators should submit statistics relating to service performance of their service unit(s) to the SWD on a quarterly basis, unless otherwise specified. The responsibility rests with Service Operators to ensure that their reported statistics are both accurate and timely. Service Operators should process and monitor the statistics to produce information about the performance of individual service unit(s) and of their service as a whole in the Service Performance Management Information System and observe all relevant submission dates as required by the SWD.

Statistical Reporting. The BCCC will provide access to FirstWatch™ to allow the Town self-service CAD research capabilities. Additionally, the BCCC can provide statistical reports from the VIPER™ 9-1-1 telephone system for the Town when requested.
Statistical Reporting. With regard to the Policies written, the PA shall report, simultaneously to the Company and to the Company’s designated statistical agent, data necessary to satisfy all regulatory bodies, including, but not limited to, reporting statistical data as required from time to time. e. Card number This information can be a computer report prepared by the City Library automation system.

Statistical Reporting. Within 7 days after the last day of each month during the Term, Design-Builder shall prepare and submit to the City a statistical report setting out:
(a) the total number of hours worked by all Design-Builder Persons on the Project Work for that month;
(b) the number of incidents for that month and the total number of incidents occurring during the Project Work;
(c) the aggregate number of lost work days in that month arising from each incident, along with a completed incident management report for each incident occurring in relation to the performance of the Project Work on the Lands, on a Site or on the Infrastructure, to the extent such incidents, near miss incidents, public safety incidents and injuries involve Design-Builder Persons or the performance of the Project Work.
