STATEMENTS REQUIRED. During the term of the Loan and while any liabilities of Borrower to Lender under any of the Loan Documents remain outstanding and unless Lender otherwise consents in writing, Borrower shall provide to Lender the following:
STATEMENTS REQUIRED. No payment will be made for Work performed by Change Directive until the Design-Builder has furnished the Owner with duplicate itemized statements of the cost of such Force Account Work detailed as follows:

(ii) Designation, dates, daily hours, total hours, rental rate, and extension for each unit of machinery and Equipment.
(iii) Quantities of Materials, prices, and extensions.

(v) Cost of property damage, liability and ▇▇▇▇▇▇▇'▇ compensation insurance premiums, unemployment insurance contributions, and social security tax. Statements shall be accompanied and supported by a receipted invoice for all Materials used and transportation charges. However, if Materials used on the changed Work are not specifically purchased for such Work but are taken from the Design-Builder's stock, then in lieu of the invoices the Design-Builder shall furnish an affidavit certifying that such Materials were taken from his/her stock, that the quantity claimed was actually used, and that the price and transportation claimed represent the actual cost to the Design-Builder.

A return on which payment of Federal Insurance Contributions Act taxes is made in accordance with this section shall have attached thereto a statement, certified by the foundation or commission filing the return, stat- ing that the foundation or commission is an organization established pursuant to an agreement made under authority of section 32(b) of the Surplus Property Act of 1944, as amended, or established or continued pursuant to an agreement made under authority of the Mutual Educational and Cultural Exchange Act of 1961, as amended.
