Common use of State Provisions Clause in Contracts

State Provisions. If a covered service is not provided to the Agreement Holder by the Administrator no later than the sixtieth (60th) day after proof of loss has been filed, or if a refund or credit is not paid before the forty-sixth (46th) day after the date on which the contract is returned to the Provider, You may apply for reimbursement directly to the reimbursement insurance company.
