STAFFINfi. 22 17.1 CONTRACTOR shall provide the following described staff positions 23 and shall submit, in a format provided and/or approved by ADMINISTRATOR, 24 monthly staffing reports to ADMINISTRATOR. Staffing Reports shall report 25 actual staff hours worked by position, and shall include the position title 26 and monthly salary and benefits. CONTRACTOR shall recruit, train and 27 supervise Care Coordinator(s), Wraparound Supervisor(s), Parent Partner(s) and 28 Youth Partner(s) in accordance with standards developed by Wraparound OC for 1 provision of Wraparound. 2 17.2 CONTRACTOR shall notify ADMINISTRATOR within twenty-four (24) 3 standard business hours when a Care Coordinator, Wraparound Supervisor, Parent 4 Partner or Youth Partner is hired, providing the name and date hired for each 5 employee, as well as qualifications and copy(ies) of diploma(s). 6 17.3 CONTRACTOR shall, for each employee, maintain an employee file 7 that contains the Care Coordinator’s, Wraparound Supervisor’s, Parent 8 Partner’s, or Youth Partner’s resume and proof of qualifications. Upon 9 reasonable notice, CONTRACTOR shall provide ADMINISTRATOR access to employee 10 files to audit for compliance with this requirement. 11 17.4 CONTRACTOR shall comply with all CDSS Community Care Licensing 12 Division regulations for firoup Home Service Providers regarding background 13 checks on all potential employees.

  • Staffing There shall be a clinician employed by the outside contractor for EAP Services who will be on-site a minimum of 20 hours a week. The clinician shall report directly to the outside contractor, Peer Assistance Oversight Committee and the MIF liaison.   • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • Orthodontics We Cover orthodontics used to help restore oral structures to health and function and to treat serious medical conditions such as: cleft palate and cleft lip; maxillary/mandibular micrognathia (underdeveloped upper or lower jaw); extreme mandibular prognathism; severe asymmetry (craniofacial anomalies); ankylosis of the temporomandibular joint; and other significant skeletal dysplasias.

  • Enterprise Information Management Standards Performing Agency shall conform to HHS standards for data management as described by the policies of the HHS Chief Data and Analytics Officer. These include, but are not limited to, standards for documentation and communication of data models, metadata, and other data definition methods that are required by HHS for ongoing data governance, strategic portfolio analysis, interoperability planning, and valuation of HHS System data assets.
