Serious Incidents and Fatalities. Investigations under this section must be carried out by persons knowledgeable about the type of work involved and, with the participation of the employer or a representative of the employer, and a worker representative selected by the local union. In addition, the company will notify the local union office and a representative of the Union shall have access to the incident site.
