SCHOOL TO WORK PROGRAM. A. The Service Provider shall hold and maintain a current Community Rehabilitation Program (CRP) contract with the Division of Vocational Rehabilitation (DVR). In the event that the Service Provider’s CRP contract is terminated, the Service Provider shall notify the County, in writing, no later than the next business day.
B. The County, DVR, and DDA will complete an intake and eligibility process for each prospective transition client, and will make a determination regarding the client’s participation in the School to Work project.
C. Following the completion of the client’s eligibly and intake process, the Service Provider shall:
1. Collaborate with the School to Work Program Coordinator, the District, DDA, DVR, and the client and their family, to identify the client’s vocational goals, and to develop and implement an Individual Plan for Employment (IPE).
2. The DVR client’s Individual Plan for Employment must include:
a. Client’s first and last name;
b. Current date;
c. Timeline for the plan;
d. Client ADSA ID;
e. Employment goal, including:
i. The preferred job type the client wishes to obtain or maintain;
ii. The preferred wages/salary the client wishes to earn, and
iii. The number of hours the individual prefers to work.
f. The client’s preferred work shift;
g. The client’s skills, gifts, interests, and preferred activities;
h. Each measurable strategy (action steps and supports) to meet the employment goal;
i. Identification of persons and/or entities (such as family, school district, Developmental Disabilities Administration, and Division of Vocational Rehabilitation) available to assist the client in reaching his/her employment goal;
j. Identification of transportation needs and supports; and,
k. Identification of other accommodations, adaptive equipment, and/or supports critical to achieve employment goal.
3. Communicate monthly with the School to Work Program Coordinator, the District, DDA, DVR, and the client and their family; keep all parties informed of timelines and progress, as outlined in the client’s Individual Plan for Employment.
4. Provide to the client with the following services: Community-based assessment, intensive job placement, and intensive training services. Such services shall be provided consistent with the content of each client’s DVR Individual Plan for Employment.
