SCHOOL TO WORK PROGRAM. A. The Service Provider shall hold and maintain a current Community Rehabilitation Program (CRP) contract with the Division of Vocational Rehabilitation (DVR). In the event that the Service Provider’s CRP contract is terminated, the Service Provider shall notify the County, in writing, no later than the next business day. B. The County, DVR, and DDA will complete an intake and eligibility process for each prospective transition client, and will make a determination regarding the client’s participation in the School to Work project. C. Following the completion of the client’s eligibly and intake process, the Service Provider shall: 1. Collaborate with the School to Work Program Coordinator, the District, DDA, DVR, and the client and their family, to identify the client’s vocational goals, and to develop and implement an Individual Plan for Employment (IPE). 2. The DVR client’s Individual Plan for Employment must include: a. Client’s first and last name; b. Current date; c. Timeline for the plan; d. Client ADSA ID; e. Employment goal, including: i. The preferred job type the client wishes to obtain or maintain; ii. The preferred wages/salary the client wishes to earn, and iii. The number of hours the individual prefers to work. f. The client’s preferred work shift; g. The client’s skills, gifts, interests, and preferred activities; h. Each measurable strategy (action steps and supports) to meet the employment goal; i. Identification of persons and/or entities (such as family, school district, Developmental Disabilities Administration, and Division of Vocational Rehabilitation) available to assist the client in reaching his/her employment goal; j. Identification of transportation needs and supports; and, k. Identification of other accommodations, adaptive equipment, and/or supports critical to achieve employment goal. 3. Communicate monthly with the School to Work Program Coordinator, the District, DDA, DVR, and the client and their family; keep all parties informed of timelines and progress, as outlined in the client’s Individual Plan for Employment. 4. Provide to the client with the following services: Community-based assessment, intensive job placement, and intensive training services. Such services shall be provided consistent with the content of each client’s DVR Individual Plan for Employment. 5.   • Modified Work/Return to Work Programs The Employer and the Union recognize the purpose of modified work/return to work programs, is to provide fair and consistent practices for accommodating nurses who have been ill, injured or permanently disabled, to enable their safe return to work. The parties undertake to provide safe and meaningful employment for all nurses based on the following objectives and principles:

  • Modified Work Program The Employer shall provide modified work for any employee injured on the job as per the requirements of the Workers’ Safety and Insurance Board. The employee must co-operate in developing and participating in, a modified work program suited to his capabilities, and with the approval of his physician and/or the WSIB. Both the Employer and the employee shall work together to return the employee to good health and his regular duties.

  • Work Progress The Developer and Connecting Transmission Owner will keep each other, and NYISO, advised periodically as to the progress of their respective design, procurement and construction efforts. Any Party may, at any time, request a progress report from the Developer or Connecting Transmission Owner. If, at any time, the Developer determines that the completion of the Connecting Transmission Owner’s Attachment Facilities will not be required until after the specified In-Service Date, the Developer will provide written notice to the Connecting Transmission Owner and NYISO of such later date upon which the completion of the Connecting Transmission Owner’s Attachment Facilities will be required.

  • Training Program It is agreed that there shall be an Apprenticeship Training Program, the provisions of which are set forth in Exhibit "D", which is attached hereto and forms part of this Agreement.

  • Work Products Grantee shall provide CalRecycle with copies of all final products identified in the Work Plan. Grantee shall also provide CalRecycle with copies of all public education and advertising material produced pursuant to this Agreement.
