Routine Care. For routine dental care detailed below, the Plan reimburses eligible employees 90% of the Ontario Dental Association Fee Guide allowance, on a one year lag basis, for the eligible expense incurred by the employee and each eligible dependent. - oral exams, including the cleaning of teeth, but not more than once every 6 months; - root planing or equilibration (limited to 8 units per year for all procedures combined); - periodontal scaling; - topical applications of sodium or stannous fluoride (where such application is necessary for the maintenance of sound dental health); - dental x-rays; - fillings; - extractions; - oral surgery, including excision of impacted wisdom teeth; - reasonable and customary charges for anesthesia and its administration; - repair, relining or rebasing of dentures; - space maintainers, including stainless steel crowns for baby teeth that have several cavities which would otherwise require filling or which are non-restorable using normal restorative dental material; -caries susceptibility test (Cariescreen); - in-office topical fluoride (Fluorzion Foam); - topical antimicrobial (Chlorzion); - dispensing of fluoride (Fluorzion Homecare); - consultants (at the start of treatment only); - microbiological test ( Bana test); - pit and fissure sealants; - periodontic treatment for disease of the bone and gums of the mouth, including tissue grafts and occlusal guards, but not athletic guards; and - endodontic treatment, including root canal therapy.
