Return or Destruction of CDPH PCI on Expiration or Termination. Upon expiration or termination of the agreement between Contractor and CDPH for any reason, Contractor shall securely return or destroy the CDPH PCI. If return or destruction is not feasible, Contractor shall provide a written explanation to the CDPH Program Contract Manager, the CDPH Privacy Officer and the CDPH Chief Information Security Officer, using the contact information listed in Section XI(F), above.
Return or Destruction of CDPH PCI on Expiration or Termination. Upon expiration or termination of the Agreement between Blue Shield and the Agency for any reason, or upon written request by the Agency or CDPH, Blue Shield shall securely return or destroy the CDPH PCI within thirty (30) calendar days. If return or destruction is not feasible, Blue Shield shall provide a written explanation to the Agency Contract Manager, the CDPH Privacy Officer and the CDPH Chief Information Security Officer, using the contact information listed in Section XII(F), above.
