REGISTRATION STATEMENT QUESTIONNAIRE. Such Purchaser has completed or will cause to be completed the Registration Statement Questionnaire attached hereto as Appendix 1, for use in preparation of the Registration Statement, and the answers thereto are or will be true and correct as of the date hereof and will be true and correct as of the effective date of the Registration Statement, or as applicable, any amendment thereto, and such Purchaser will notify the Company immediately of any material change in any such information provided in the Registration Statement Questionnaire until such time as the Registration Statement has been declared effective (and thereafter at the Company's reasonable request in connection with preparation of a prospectus supplement, amendment to the Registration Statement, or otherwise).
