Rate Change Explanation. If an Individual’s rate increases or decreases based on change of need as identified in a reassessment, the Provider can ask for an explanation of the change(s) made by the reassessment. The request must be made in writing to the DHS/OHA designated contact within thirty (30) days of the completed assessment. DHS/OHA will respond within two (2) weeks of the written request.
