Quantity Per Dispensing Limits/Allowances are based on the following: (a) the manufacturer’s recommended dosage and duration of therapy; (b) common usage for episodic or intermittent treatment; (c) FDA-approved recommendations and/or clinical studies; or (d) as determined by Us.
