Psychological Evaluation. Through the use of a variety of standard psychological tests, we will attempt to answer the questions that have brought you or your child for this assessment. These questions generally concern difficulties in the areas of socialization, behavior, learning, academic functioning, emotional functioning, or coping. Throughout the assessment process you and/or your child have the right to inquire about the nature or purpose of all procedures. You also have the right to know the test results, interpretations, and recommendations. The first step of the assessment process involves an informational interview (the intake). This will be the first session and should be attended by you or you and your child if your child is the client. After this interview, the psychologist will determine whether it is in the best interest of the client to move forward with the evaluation process. If they decide to move forward, additional testing sessions will be scheduled after preauthorization from insurance is obtained (if needed). These testing sessions are comprised of the administration of one or more psychological and/or educational tests. Although it is sometimes possible to complete the testing procedure in one sitting, it is more common for people to be asked to return for additional sessions to finish the assessment battery. This is especially true when evaluating children. Once testing is completed, the data will be analyzed and a report will be written. You will then have the opportunity to meet with your provider to discuss the results and receive a copy of the report. The information obtained in the evaluation is confidential and will not be released to any person or organization without your written permission. (This release is available in our office or may be completed with any individual whom you wish to give such access, and then provided to us.) The only exceptions to this policy are rare situations in which your provider is required by law to release information with or without your permission. Please refer to the upcoming section entitled ‘Confidentiality.’ Purposes for psychological evaluation are multiple and may include, but are not limited to, diagnostically classifying an individual. It is important to understand that a psychological evaluation may serve this purpose but is not guaranteed. There are factors involved in psychological evaluation that may affect whether a diagnosis can be given at that time (e.g., a patient’s compliance with testing procedures...
Psychological Evaluation. During the course of this Evaluation/Investigation, I will not give an opinion about the psychological functioning of any individual, or clinically diagnose any individual, without a professionally conducted psychological evaluation. If I become concerned about the psychological or emotional functioning of any individual, and have not received a psychological evaluation for that individual, I may request that the Court order such an evaluation by a qualified professional before I can complete my Evaluation/Investigation. If such an evaluation is not ordered by the Court, or is otherwise unavailable, I may include a statement of my concerns in my written summary.

Psychological Evaluation. If Defendants elect to subject applicants for employment and/or current employees to Psychological Evaluation, Defendants will establish and follow a written procedure which complies with all requirements of the ADA, including the following specific provisions relating to medical examinations and inquiries: A. Before Defendants make a Conditional Offer of employment, Defendants may not subject any applicants for employment to Psychological Evaluation. Defendants may not make inquiries that would tend to elicit information about any Mental Disorder. B. After a Conditional Offer of employment has been made but prior to the commencement of employment, Defendants may subject applicants to Psychological Evaluation only if such testing is strictly limited to the determination by Individualized Assessment of whether the individual, with or without a Reasonable Accommodation, (a) is able to perform the Essential Functions of the position in question or (b) poses a Direct Threat. C. With respect to current employees, Defendants may not adopt or implement any policy or practice of subjecting employees to Psychological Evaluation as a condition of continued employment. With respect to individual employees, employers may not subject an employee to Psychological Evaluation unless reliable, objective and current evidence indicates that the individual, with or without a Reasonable Accommodation, (a) is unable to perform the Essential Functions of his or her job or (b) poses a Direct Threat. Any Psychological Evaluation that is conducted on this basis must remain strictly limited to the determination by Individualized Assessment of whether the individual, with or without a Reasonable Accommodation, (a) is able to perform the Essential Functions of the position in question or (b) poses a Direct Threat. D. Any records relating to Psychological Evaluation must be maintained separately from the applicant or employee’s personnel file and must be treated as a confidential medical record.
Psychological Evaluation. All forensic psychological services are billed at a rate in accordance with my fee schedule. Charges are calculated in 15 minute increments. Travel Time. All travel to and from court proceedings, depositions and psychological evaluations are billed at $50 per hour. Where I am required to travel over 200 miles for any court proceeding, deposition or evaluation, whether it be in state or out of state, you shall cover the costs of my accommodations and make hotel arrangements in advanced. Out of pocket expenses. I also ▇▇▇▇ for out of pocket expenses, such as copying, conference room rental, telephone calls, overnight delivery, courier services and other administrative tasks. Copies will be charged at $0.10 per page. All administrative tasks will be charged at the out of pocket cost.
Psychological Evaluation. SCHOOL BOARD shall arrange to have its employee guardian candidates who volunteer to become school guardians to complete a psychological evaluation administered by a psychologist licensed under chapter 490, Florida Statutes, and designated by the Department of Law Enforcement and submit documentation of the results of the evaluation to the Sheriff.
