Common use of Program Reporting Clause in Contracts

Program Reporting. A. Utilize the HMIS ClientTrack “CoC Intake Workflow” and the “CoC Exit Workflow” for all SHP clients to capture the HUD-required Universal Data Elements and Program Descriptors based on current HMIS Data Standards B. Provide quarterly ClientTrack reports in preparation for the HUD Annual Progress Report (APR) C. Provide ClientTrack additional reports as requested by HHSA staff to fulfill obligations of HUD reporting
