Common use of Program Reporting Clause in Contracts

Program Reporting. 2.11.2.1. Grantee is required to enter or upload data into the ILS by the fifth day of the following month in accordance with the user guide instructions. The ILS Data Reporting System captures program performance and financial data about each ILS Program contract, including data on waiting lists, monthly individual cost share participation, financial information, goals met and unsuccessful case closures, and purchased service records for individuals served. 2.11.2.2. HHSC will use information in this data reporting system to monitor the ILS Program contracts, key performances measures, and work plan targets. 2.11.2.3. By the fifth day after the end of the previous month, Grantee will be required to enter success stories for individuals served into the ILS Data Reporting System. These success stories should include examples of individuals served with different disabilities, including individuals who are deaf or hard of hearing and blind or visually impaired.
