Program Referrals. The only individua ls who may participate in the Consultant’s programs under this Agreement are those who have been referred to the program by the County or who are eligible for the program based on criteria approved by the County. All referrals must be submitted on approved County forms. For all program services, Probation will contact minor and family participants to notify them of referral to services. Consultant shall contact family of referred minor and report the status of contacts within ten (10) business days of Probation’s referral form. Participation of any individua l not referred by the County or through the procedures established herein shall result in the disallowance of the Consultant’s costs associated with the participation of that individua l in Consultant’s program.

Program Referrals. Per the Commercial Terms outlined below, VH and AGW will refer each other to possible customers internationally who may want only the partner’s product. Based on this MOU Agreement, VH and AGW will conduct specific projects related to the purpose and objective of this MOU Agreement, subject to subscription of subsequent Addendums, which will include the following specific elements: • General description of the project • Objective of project • Promotion and implementation strategies • Specific goal(s) and action plan(s), including costs and expected income • Resources assigned to the project • Compensation structure • Legal governance
Program Referrals. Grantee shall offer appropriate referrals to all intervention participants. Referrals may include but are not limited to: HIV/HCV and STD testing; medical care; housing; transportation; mental health; food and nutrition; syringe service programs; PrEP access; PrEP Medical Assistance; and, HIV testing. Grantees providing HR and SSP services through this funding must offer PrEP and HIV testing referrals.

  • Program Review The Contracting Officer or other authorized government representative may hold semi- annual program review meetings. Such meetings will be held via telecom or video teleconferencing. However, the Government reserves the right to request a meeting in person. The meetings will include all BPA holders, representatives from prospective customer agencies, a combination of current and prospective customer agencies, or individual BPA holders. Some Federal Government Agencies and any approved State, Local and Tribal agencies may establish a central program management function. Such users may require their primary suppliers to participate in agency program review meetings on a periodic basis, at no additional cost to the Government.

  • Program Overview A. Goals of the Choice Neighborhoods Program. The Choice Neighborhoods Program employs a comprehensive approach to neighborhood transformation. The program transforms neighborhoods of concentrated poverty into mixed-income neighborhoods of long-term viability by revitalizing severely distressed public and/or assisted housing; improving access to economic opportunities; and investing and leveraging investments in well-functioning services, effective schools and education programs, public assets, public transportation, and improved access to jobs. Choice Neighborhoods ensures that current residents benefit from this transformation by preserving affordable housing in the neighborhood or providing the choice to move to affordable housing in another neighborhood of opportunity. The purpose of this grant is to implement a Transformation Plan that has been developed through a local planning process and furthers the goals of the Choice Neighborhoods Program. The core goals of Choice Neighborhoods are:

  • Program Evaluation The School District and the College will develop a plan for the evaluation of the Dual Credit program to be completed each year. The evaluation will include, but is not limited to, disaggregated attendance and retention rates, GPA of high-school-credit-only courses and college courses, satisfactory progress in college courses, state assessment results, SAT/ACT, as applicable, TSIA readiness by grade level, and adequate progress toward the college-readiness of the students in the program. The School District commits to collecting longitudinal data as specified by the College, and making data and performance outcomes available to the College upon request. HB 1638 and SACSCOC require the collection of data points to be longitudinally captured by the School District, in collaboration with the College, will include, at minimum: student enrollment, GPA, retention, persistence, completion, transfer and scholarships. School District will provide parent contact and demographic information to the College upon request for targeted marketing of degree completion or workforce development information to parents of Students. School District agrees to obtain valid FERPA releases drafted to support the supply of such data if deemed required by counsel to either School District or the College. The College conducts and reports regular and ongoing evaluations of the Dual Credit program effectiveness and uses the results for continuous improvement.

  • Technical Support Services 2.1 The technical support services (the "Services"): Party A agrees to provide to Party B the relevant services requested by Party B, which are specified in Exhibit 1 attached hereto ("Exhibit 1").

  • Allowable Customizations The Student is permitted to alter or add files to customize the assigned Computer to her/his own working style (i.e., background screens and images, display settings).
