PROGRAM PERFORMANCE REPORTS. The parties to this agreement shall monitor the performance of the Good Neighbor Agreement activities to ensure that performance goals are being achieved. Performance reports shall contain information on the following: - A comparison of actual accomplishments to the goals established for the period. The output of the project can be readily expressed in numbers, a computation of the cost per unit of output, if applicable, - Reason(s) for delay if established goals were not met. The State shall submit annual performance reports. These reports are due 30 days after the reporting period ending June 30. The final performance report shall be submitted either with the State’s final payment request, or separately, but not later than 90 days from the expiration date of the Good Neighbor Agreement.
