Common use of Program Coordination Clause in Contracts

Program Coordination. a. The Manager shall monitor ▇▇▇▇▇▇▇’s progress and performance of the Program. All services agreed to be performed by Grantee shall be under the general direction of the Manager. b. Grantee shall assign a single Program manager (“Grantee Manager”) who shall have overall responsibility for the performance of the Program and compliance with this Agreement by Grantee. Should circumstances or conditions require a substitute Grantee Manager, Grantee shall notify the Manager immediately of such occurrence. ▇▇▇▇▇▇▇'s staff shall cooperate fully with the Manager with respect to all matters related to this Agreement. c. ▇▇▇▇▇▇▇’s staff shall attend and actively participate in the monthly Santa ▇▇▇▇ Violence Prevention Partnership (“The Partnership”) Operational Team, the monthly Multi- Disciplinary Assessment and Referral Team (“MDART”), and other meetings as required or requested by the Manager, including meetings described elsewhere in this Agreement. 1. Grantee shall designate one primary and one secondary representative to attend meetings required or requested by the Manager. If the primary representative cannot attend a required meeting, Grantee shall send in their place the secondary representative. In no CHOICE Grant Funding Agreement -Cycle IX Year 2 Form approved by the City Attorney 11/5/2018 event shall a required meeting be unattended by Grantee without prior authorization from the Manager. Required meetings include but are not limited to the monthly Operational Team and MDART meetings. 2. Active participation in the monthly MDART meetings consists of providing updates on any youth referred through the Guiding People Successfully Program, conferring with MDART meeting attendees when ▇▇▇▇▇▇▇’s participant(s) are in need of additional services not provided by Grantee, providing feedback as requested, and responding to City requests. d. ▇▇▇▇▇▇▇’s staff shall actively participate with The Partnership’s annual Gang Prevention Awareness Month in collaboration with the City, as requested by the Manager. e. ▇▇▇▇▇▇▇’s staff shall prioritize any client referrals submitted to The Partnership that pertain to the services provided by Grantee under the terms of the Agreement. f. Grantee shall incorporate The Partnership’s logo and the phrase “Funded by Measure O” on any promotional and marketing materials related to the Program. g. Program Coordination. a. The Manager A. Each year in August during the term of this Agreement, USER shall monitor provide City with a draft annual program plan for City’s approval, Upon approval of the annual program by the CITY REPRESENTATIVE, USER (or User’s approved subcontractors) shall offer the Community Services described in the Program to the public in accordance with the approved written plan which plan is further described on the Exhibit A Scope of Services. B. NOTE TO PROPOSERS – THIS SUBSECTION B ONLY APPLIES TO NORTHSIDE USER and also acknowledges that USER shall comply with the requirement that the Center shall be made available to organizations, businesses and residents located in the Japantown Redevelopment Area no less than 45 days per year, which is set forth in a use covenant required by the Redevelopment Agency of San ▇▇▇▇▇▇▇’s progress and performance ▇ as a condition to its contribution toward the construction costs for the Center. USER shall include written details in the draft annual plan of the Program. THIS PROVISION MAY BE MODIFIED BY CITY DEPENDING UPON EXTENT OF USER’S PROPOSAL AND OPERATIONS - USER represents that USER shall employ, at its sole cost and expense, [a full-time (40 hours per week) administrator] who shall act as the director of the Manager. b. Grantee shall assign a single Program manager (“Grantee Manager”) who and shall have overall responsibility for the performance progress and execution of this Agreement, as well as supervise the operation of the Program. USER shall provide CITY with the contact information for USER’s Program and compliance with Director upon execution of this Agreement by GranteeAgreement. Should circumstances or conditions subsequent to the execution of this Agreement require a substitute Grantee ManagerProgram director, Grantee USER shall notify the Manager CITY REPRESENTATIVE immediately of such occurrence. ▇▇▇▇▇▇▇USER 's staff shall cooperate fully with the Manager CITY REPRESENTATIVE with respect to all matters related to this Agreement. c. ▇▇▇▇▇▇▇D.
