Post-Release Plan. For clients in custody, Contractor shall assess and screen the client using an established tool and help the client develop a post-release plan, which includes a plan to address immediate needs after release from custody. The plan may include transportation, location/date of first appointment, identified housing resources, and other immediate needs. The post-release plan should be documented and kept for records.
