Plan Choice Clause Samples

Plan Choice. Eligible employees including spouse and dependents, may participate in the Base Health Insurance Plan offered to district employees. The employee will contribute twenty percent (20%) towards premium of the base plan offered by the District.
Plan Choice. During open enrollment periods determined by the Town, employees may select the following coverages with the following copays effective upon ratification: 1. Anthem Blue Cross and Blue Shield Century Preferred Non-Standard Plan as follows:
Plan Choice. Eligible employees including spouse and dependents, may participate in the Capital District Physician’s Health Plan program offered to district employees. The employee will pay 15% toward the premium in the 2014-2015 school year, 17.5% in the 2015-2016 school year, and 20% in the 2016-2017 school year.
Plan Choice. Eligible employees including spouse, domestic partner and dependents, have a choice of Capital District Physician’s Health Plan (CDPHPP and Blue Shield Point of Service). The employee will pay 15% toward the premium.
