Common use of Physician Certifications Clause in Contracts

Physician Certifications. A pregnant employee who elects not to apply for an unpaid pre-childbirth leave pursuant to Section 4 a. above shall be permitted to continue on active duty until such date as the employee and their physician determine that they must be absent due to pregnancy disability, provided that they can and do continue to perform the full duties and responsibilities of their position. The employee must also supply to the District their physician's certification as to the beginning and ending dates of actual pregnancy-related disability for which sick leave is claimed, and their physician's release to return to active duty.

A pregnant employee unit member who elects not to apply for an unpaid pre-childbirth leave pursuant to Section 4 a. paragraph l above shall be permitted to continue on active duty until such date as the employee she and their her physician determine that they she must be absent herself due to pregnancy disability, provided that they she can and do does continue to perform the full duties and responsibilities of their her position. The employee unit member must also supply to the District their her physician's ’s certification as to the beginning and ending dates of actual pregnancy-related disability for which sick leave is claimed, and their her physician's ’s release to return to active duty.
