Performance/Progress Report Requirements. At the end of each quarter, the Grantee is responsible for quarterly reporting. This includes completion of violation determinations, documentation of enforcement and follow-up actions on violations, sanitary survey updates, and other required WT data entry. Deadline is no later than 15 days following the end of the quarter. After WT data entry is reviewed by the State, a payment request will be processed (see F. Reimbursement Schedule below).
