PERFORMANCE OUTCOMES. CONTRACTOR’s services shall enable Clients to 23 adaptively function at a higher and more appropriate level of independence. 24 1. Fifty percent (50%) of Clients placed in CONTRACTOR’s Enhanced Residential 25 Rehabilitation Program will remain out of the hospitals or long-term care facilities for a minimum of six 26 (6) months. 27 2. Fifty percent (50%) of Clients placed in CONTRACTOR’s Enhanced Residential 28 Rehabilitation Program will be able to move to a lower level of care within eighteen (18) months. 29 3. CONTRACTOR shall report the status of these outcomes on a quarterly basis, and include 30 the following information: number of Clients placed, date of placement for each Client, number 31 of hospitalizations for each Client, number of discharges to a lower level of care for each Client, and 32 length of stay in the program for each Client.
