Performance Evaluation Reports. A. Performance evaluation reports shall be made on forms developed by the District in consultation with the Union and shall be prepared by the worker’s immediate manager. The form may be reviewed by the next higher manager and, if the need for improvement is noted, by the department manager, if appropriate.

Performance Evaluation Reports. Extension Lecturers who have completed the 8 Probationary Period shall submit a Performance Appraisal System (PAS) Annual Report 9 by the end of the annual review cycle in Autumn Quarter (Autumn of previous year 10 through Summer of current year).
Performance Evaluation Reports. Prior to the anniversary date or pay increase eligibility, the Department Head shall review the performance record of the employee under consideration and based upon this review, forward a recommendation of approval or denial to the City Manager. All recommendations for denial of a step increase must be accompanied by a statement of specific reasons which shall be placed in the employee's record. If an employee is denied a step increase, a re-evaluation shall be conducted in three (3) months. The employee may request the re-evaluation to occur sooner. If improvement in performance is noted, the step increase shall be granted.
Performance Evaluation Reports. There shall be a formal written evaluation system and rating of each supervisor completed at least annually. Evaluations shall be made at least once each year for supervisors. A supervisor must have worked for their immediate supervisor for at least three months before an evaluation can be performed.

  • Performance Review Where a performance review of an employee’s performance is carried out, the employee shall be given sufficient opportunity after the interview to read and review the performance review. Provision shall be made on the performance review form for an employee to sign it. The form shall provide for the employee’s signature in two (2) places, one (1) indicating that the employee has read and accepts the performance review, and the other indicating that the employee disagrees with the performance review. The employee shall sign in only one (1) of the places provided. No employee may initiate a grievance regarding the contents of a performance review unless the signature indicates disagreement. An employee shall, upon request, receive a copy of this performance review at the time of signing. An employee’s performance review shall not be changed after an employee has signed it, without the knowledge of the employee, and any such changes shall be subject to the grievance procedure of this Agreement. The employee may respond, in writing, to the performance review. Such response will be attached to the performance review.

  • Program Evaluation The School District and the College will develop a plan for the evaluation of the Dual Credit program to be completed each year. The evaluation will include, but is not limited to, disaggregated attendance and retention rates, GPA of high-school-credit-only courses and college courses, satisfactory progress in college courses, state assessment results, SAT/ACT, as applicable, TSIA readiness by grade level, and adequate progress toward the college-readiness of the students in the program. The School District commits to collecting longitudinal data as specified by the College, and making data and performance outcomes available to the College upon request. HB 1638 and SACSCOC require the collection of data points to be longitudinally captured by the School District, in collaboration with the College, will include, at minimum: student enrollment, GPA, retention, persistence, completion, transfer and scholarships. School District will provide parent contact and demographic information to the College upon request for targeted marketing of degree completion or workforce development information to parents of Students. School District agrees to obtain valid FERPA releases drafted to support the supply of such data if deemed required by counsel to either School District or the College. The College conducts and reports regular and ongoing evaluations of the Dual Credit program effectiveness and uses the results for continuous improvement.

  • Quarterly Contractor Performance Reporting Customers shall complete a Contractor Performance Survey (Exhibit I) for each Contractor on a Quarterly basis. Customers will electronically submit the completed Contractor Performance Survey(s) to the Department Contract Manager no later than the due date indicated in Contract Exhibit D, Section 17, Additional Special Contract Conditions. The completed Contractor Performance Survey(s) will be used by the Department as a performance-reporting tool to measure the performance of Contractors. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MyFloridaMarketPlace or on the Department's website).
