Common use of OPTICAL Clause in Contracts

OPTICAL. “Effective January 1, 2019, eligible employees will be reimbursed up to eighty (80%) percent of the cost of frames, lenses, and the fitting or prescription eye glasses when recommended by a doctor or optometrist up to a maximum of two hundred ($200) dollars in each two (2) year period of active employment. The cost of prescription contact lenses will be covered in the same amount. Eligible employees must have five (5) years of continuous service and must have worked a minimum of six hundred (600) hours in each of the two calendar years preceding any reimbursement. This plan provides employee coverage only. DRUG PRESCRIPTION Effective, January 1, 2019, eligible employees will be reimbursed one hundred percent of prescription drugs to a yearly maximum of $1000 (one thousand) dollars. Eligible employees must have five (5) years of continuous service and must have worked a minimum of six hundred (600) hours in each of the two (2) calendar years preceding any reimbursement. This plan provides single coverage only.
