OHIP. 21.01 Effective January 1, 1989, the Company shall make reimbursement of seventy-five (75%) percent of the cost of single coverage, or in the case of a single parent, dependent coverage, to part-time employees with five (5) years' continuous service and who work six hundred (600) hours in the previous calendar year, with payments to be made quarterly upon proof of purchase.
