Common use of Medical Statements Clause in Contracts

Medical Statements. Upon a Teacher's return to work from an absence of more than three (3) consecutive days, for the Teacher's illness or family illness, the immediate supervisor may request that the employee provide a medical statement from a physician to the District’s Privacy Officer.
