Common use of Lakes District Health Board Clause in Contracts

Lakes District Health Board. After every 10 years of continuous service the employee will be entitled to a long service holiday of two weeks. Such leave is to be taken in one continuous spell within five years of qualifying. Leave not taken within the five year period will be forfeited providing that six months notice of pending forfeiture is given. Employees who are eligible for long service leave at the date of the retirement or resignation are entitled to equivalent salary in lieu of leave. The employer will pay a cash payment equivalent to two weeks salary to the surviving spouse or dependent child(ren) or the estate of a deceased employee who had qualified for but neither taken nor forfeited long service leave.
