Individualized Services. Services tailored to the specific, unique needs of Participant and/or Participant’s family; incorporating a flexible, creative approach to treatment planning based on an assessment of needs, resources, and family strengths; and including the use of formal and informal supports and services.
Individualized Services. Participate with project partners on identifying how ArtCIE program participants will be provided information and support services to assist in exercising informed choice regarding CIE options, including but not limited to participation in on-the-job training, job shadowing, and internships, to ensure that opportunities are identified based on ArtCIE program participants' choices and experiences toward achieving CIE outcomes.
Individualized Services. In order to meet eligibility, the client must be enrolled with a Community Behavioral Health Services Provider and meet criteria below: 1. Client meets the definition of an adult with Serious Mental Illness (SMI) or Substance Use Disorder (SUD) 7 AAC 160.990. 2. Client is at risk of institutional or residential care due to any ONE of the following: a. Psychiatric instability related to mental illness or behavioral instability related to substance use b. Housing instability or homelessness c. Medical instability d. Incarceration within the previous 12 months e. Current residence in an assisted living facility In order to meet eligibility, the client must be enrolled with a Community Behavioral Health Services Provider and meet criteria noted below: 1. Client meets the definition of an adult with Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD) 7 AAC 160.990. 2. Client is currently participating in the HUD Section 811 program. In addition, the Provider must a) have the organizational capacity to provide a full range of permanent supportive housing services to the client and b) have one or more staff that has at minimum some basic training and/or experience providing permanent supportive housing services. In order to meet eligibility, the client must be enrolled with a Community Behavioral Health Services Provider and meet the criteria below: 1. Client meets the definition of an adult with Serious Mental Illness (SMI) or Substance Use Disorder (SUD) 7 AAC 160.990. 2. Client demonstrates a history of an inability to obtain or maintain competitive employment as a result of their disability. In addition, the Provider must a) have the organizational capacity to provide a full range of supported employment services to the client and b) have one or more staff that has at minimum some basic training and/or experience providing supported employment services.
Individualized Services. 2nd paragraph South Fork will facilitate the services and provide on-site assistance for the services. Kernville will be billed by South Fork for the cost of the speech services provided to Kernville students as invoiced to South Fork from Tiny Eye. The parties stipulate that placement of a Kernville student in the special day class on the grounds of a South Fork school is not an interdistrict transfer.

Client meets the definition of an adult with Serious Mental Illness (SMI) or Substance Use Disorder (SUD) 7 AAC 160.990. 2. Client is at risk of institutional or residential care due to any ONE of the following: a. Psychiatric instability related to mental illness or behavioral instability related to substance use b. Housing instability or homelessness c. Medical instability d. Incarceration within the previous 12 months e. Current residence in an assisted living facility 1. Client must be 21-24 years of age. 2. Client must be currently enrolled and receiving services in a DBH grant funded TIP or TAY program.
