IMPORTANT RESTRICTIONS. You must elect to participate prior to the beginning of each plan year. There is no allowance for late enrollment. * The amount which you designate for medical reimbursement may not subsequently be used for reimbursement of dependent day-care expenses and vice versa. * If you do not file sufficient claims for reimbursement, you will lose the unused amounts. This is often referred to as the “use it or lose it” rule. * During an unpaid leave of absence, contributions to the unreimbursed medical account must be made on an after tax basis just like any insurance premiums. When the employee returns to work, the pre-tax contribution will resume.
IMPORTANT RESTRICTIONS a. No Monetary Value: Points have no monetary value, and can only be used to claim Rewards under the Program.
IMPORTANT RESTRICTIONS. Employees are forbidden from using the Company-provided Internet access for personal business, for personal charitable endeavours or for amusement/entertainment purposes. Employees are reminded that access to the Internet should never create either the appearance or the reality of inappropriate use. Employees must not access, download, use, share, store, forward, print, communicate or create information that is unethical, illegal, prohibited or inappropriate. Although not an exhaustive list, prohibitions include sites, images, messages or materials that are obscene, pornographic, sexual, racial, gambling or drug related, violent, defamatory, derogatory, discriminatory or harassing in nature. WHAT IF SOMEONE VIOLATES THIS POLICY: Manulife reserves the right to terminate the employment (on a for cause basis) or the business relationship with any individual where that individual is in violation of the important restrictions identified in # 3 above. Manulife Financial will not defend or indemnify any employee or individual being sued or prosecuted in connection with any inappropriate use of the Internet privileges. Manulife Financial retains the right to remove from its information systems any material it views as offensive or potentially illegal. Other policy violations may also result in disciplinary action, up to and including dismissal or termination of relationship.
IMPORTANT RESTRICTIONS. A. Subject to the terms and conditions of this agreement you are granted a limited transferable, reservation and/or gift certificate to use at Skydive the Ranch in Gardiner, NY. Ownership of reservation/gift certificate may be transferred to another person at any time. B. You agree that reservation deposits are non-refundable. However, in the event there is inclement weather on the day of the reservation, the jumper may request, on the reservation day, a refund of reservation deposit. This does not apply to gift certificate holders. C. You agree that you must give 48 hours notice to change or cancel your reservation. Any changes made within 48 hours of your appointment may result in loss of reservation deposit. D. You agree that gift certificate purchases are non-refundable. The recipient of the gift certificate may transfer the gift certificate to another person. The weather related reservation refund policies do not apply to gift certificates.
