Assign a Review Hearing Committee composed Committeecomposed of 3-5 members of the Coalition Board within 3 business days. b. Respond to Request for Hearing Review to the provider in writing, return receipt requested within 5 business days. The notice must include:Board Chair will appoint the Review Hearing Committee and the committee chair, to include at least one mandated board member & one provider representative . If neither provider representative is available, this requirement will be waived and the minutes of the Review Hearing Committee will document every attempt to have a provider representative member included.
