Housing Focused Services Clause Samples

The 'Housing Focused Services' clause defines the requirement for service providers to deliver support specifically aimed at helping individuals obtain and maintain stable housing. This typically involves activities such as assisting clients with housing applications, connecting them to landlords, providing tenancy education, and supporting them in overcoming barriers to housing, such as credit issues or lack of documentation. The core function of this clause is to ensure that the services provided are directly aligned with the goal of securing permanent housing for clients, thereby addressing homelessness or housing instability in a targeted and effective manner.
Housing Focused Services. Shelter Providers are expected to engage participants in a wide range of service needs, including, but not limited to: employment/benefits, health, substance use, mental health, legal issues and transportation. Program staff should regularly engage participants on how these various other service areas are in support of their overall housing goal and allow these providers to meet with participants on the shelter site. Housing must be the primary focus of shelter staff.
Housing Focused Services a. Navigation Center staff will provide housing case management to residents that is driven by the needs of the resident, is flexible, uses a strengths-based approach and is focused on obtaining and maintaining housing. Navigation Center staff will connect shelter residents to mainstream services including but not limited to behavioral health, chemical dependency, education or workforce training, employment services and permanent supportive housing. Navigation Center staff will begin housing planning when guests are settled, and will include regular meetings with housing stabilization navigators who will work to identify guests' self-identified housing goals and leverage their strengths and assets in combination with community resources and programs. Navigation Center staff will utilize trauma-informed practices at the shelter, including: none time-limited stays, 24/7 accessibility to the shelter facility, on- site services from specialist providers whenever possible (including physical health and behavioral health supports), an overall focus on "screening people in" and supporting their staying sheltered until the appropriate next housing destination is available and accessible.

Snohomish County and subcontractors will connect shelter residents to mainstream services including but not limited to behavioral health, chemical dependency, education or workforce training, employment services and permanent supportive housing. Snohomish County and subcontractors will offer services that are trauma- informed, strengths based and culturally relevant. Services include housing search/placement.

Housing Resource Center will connect shelter residents to mainstream services including but not limited to behavioral health, chemical dependency, education or workforce training, employment services and permanent supportive housing. Housing Resource Center will have case management services available on site twice weekly. In addition, Housing Resource Center staff will invite other community supports on site, including a Veterans Benefit Specialist and landlord liaisons from Reliable Enterprises and Gather Church.
