GENERAL TERMS & CONDITIONS. ● The Device is and will remain at all times the property of Deer Creek-Mackinaw CUSD 701, and is being loaned for educational purposes only. Deer Creek-Mackinaw CUSD 701 may take back the Device, or place additional restrictions on the student’s use or possession of the Device, at any time and for any reason, with or without prior notice. If instructed to do so for any reason by any Deer Creek-Mackinaw CUSD 701’s teacher or administrator, the student or the student’s parent/guardian will immediately surrender the Device to that teacher or administrator. ● At its sole discretion, Deer Creek-Mackinaw CUSD 701 may change the terms or conditions of the student’s possession or use of the Device, or to impose new restrictions on use or possession of the Device, at any time during the term of this agreement. ● The student’s possession and use of the Device will be subject at all times, both on and off campus, to the terms and conditions described in this document, any and all applicable state and federal laws and regulations, applicable Student handbook policies (including the Technology Acceptable Use Policy) as well as any additional rules, regulations, and restrictions that may be imposed from time to time by Deer Creek-Mackinaw CUSD 701. Any included software may be used only in accordance with the applicable license and it is the student/parent’s responsibility to be familiar with and to comply with the provisions of any such license. ● By signing this agreement, the student and the student’s parent/guardian certify that they have reviewed and understand the Student Handbook and the Technology Acceptable Use Policy contained therein. They further certify that it is their responsibility to inform themselves as to any school policies that might apply to the student’s use or possession of the Device and to comply with those policies at all times. ● By signing this document, the student and the student’s parent/guardian acknowledge that they are solely responsible for ensuring that the student’s use of the Device to access the internet while off campus will be safe and responsible and in compliance with all applicable laws, policies, rules, and regulations. Deer Creek-Mackinaw CUSD 701 will not be responsible for any harm that may come to the student or any other person as a result of the student’s off-campus internet activities. ● Any violation of the terms or conditions set forth or referenced in this document may result in Deer Creek-Mackinaw CUSD 701 taking back the Device, or restricting, suspending, or terminating, with or without prior notice, the Student’s use of the Device. ● The student’s possession and use of the Device is a privilege, not a right. By signing this document, the student and the student’s parent/guardian acknowledge that they have no right or entitlement to possession or use of the Device and that neither this document nor any conversation, correspondence, or understanding between themselves and any representative of the Deer Creek-Mackinaw CUSD 701 gives them any ownership of any kind whatsoever in the Device. Prior to obtaining a device, students must… ● Pay an annual technology fee of $50.00 by check or money order made payable to Deer Creek-Mackinaw CUSD 701 (unless the technology fee has been waived or reduced by Deer Creek-Mackinaw CUSD 701 for economic hardship.) ● Sign the Technology Acceptable Use Policy ● Sign the Device Agreement (this document) Issuance of Device and other equipment ● After the annual technology fee has been paid in full (or waived or reduced for economic hardship) the Device and adapter will be issued to the student. ● For freshman, sophomores and juniors, Devices and adapters must be turned in by the last day of attendance. The specific date, and procedures for when and where to turn the units in will be communicated on the school website. ● For seniors, the Devices and adapters must be turned in by the specified return date, which is typically during the last week of classes for seniors. The specific date, and procedures for when and where to turn the units in will be communicated on the school website. ● If, for any reason, a student needs use of the Device beyond the check-in date, they must request an extension from the technology director. ● When the Device and adapter are returned, they will be checked for damages, and fees assessed (if necessary) based on the schedule listed in the “Fees” section in this document. ● Failure to turn in any equipment by the specified due date will result in late fees, as described in the “Fees” section in this document. ● Students MUST turn in the Device and adapter that they received at the start of the year. Items will be treated as “missing” if they are not the number that was issued. ● Alternative items not purchased by Deer Creek-Mackinaw CUSD #701 to replace lost or stolen equipment (such as a replacement power adapter) will not be accepted, and students will be charged for the missing equipment.

They further certify that it is they are aware of Board policies available on system’s website and their responsibility to inform themselves as to any school policies that might apply to the student’s use or possession of the Device device and to comply with those policies at all times. ● By signing this document Under no circumstances will students use device or permit it to be used in furtherance of any following: crime; fraud; threat; defamation; plagiarism; copyright, patent, or trademark infringement; illegal downloading; theft of intellectual property; gambling; accessing, viewing, or transmission of pornographic or violent images or content; illegal or unauthorized accessing or use of data; bullying or harassment (including cyber-bullying); malicious internet activities (including “hacking” of other computers or websites); advertising or commercial activities; abusive or insulting communications; or any unlawful activities of any kind.  Under no circumstances will the student use device, or permit device to be used, to access any networks, websites, or online resources not approved and the student’s will follow all current internet filtering policies.  Under no circumstances will device be used for instant messaging (IM) j, visiting chat rooms, and/or non-school social networking websites unless specifically approved by MCS. Parents or guardians will supervise and monitor internet use whenever possible.  Student and parent/guardian acknowledge that they are solely responsible for ensuring that the student’s use of the Device to access the internet while off campus will be safe and responsible and in compliance with all applicable laws, policies, rules, and regulations. Deer Creek-Mackinaw CUSD 701  Student and parent/guardian will not be responsible hold MCS and employees harmless for any harm that may come to the student or any other person as a result of the student’s off-campus internet activities.  Should the student inadvertently gain access through the mobile device to any confidential information about students or staff members, including but not limited to course work or grade information, the student will immediately report the incident to the school principal and will not share information with any person?  If authorized, by MCS the student may transport device to and from home, public libraries, and meetings of school study groups.  Student will not share device with any person unless expressly authorized to do so by a MCS teacher or administrator.  Student will not loan the device to any person, including another MCS student, without prior written authorization from the principal.  Any violation of the terms or conditions set forth or referenced in this document may result in Deer Creek-Mackinaw CUSD 701 taking back the Devicepossession or use of device being restricted, suspended, or restricting, suspending, or terminatingterminated, with or without prior notice, at the sole discretion MCS.  Student’s use of the Device. ● The student’s possession and use of the Device device is a privilege, not a right. By right and by signing this document, the student and the student’s parent/guardian acknowledge that they have no right or entitlement to possession or use of the Device device and that neither this document nor any conversation, correspondence, correspondence or understanding between themselves and any representative of the Deer Creek-Mackinaw CUSD 701 MCS, gives them any ownership or contractual rights of any kind whatsoever to the device. Orientation Meeting, Technology Fee, & Quiz  As a condition of being issued a mobile device, all participants in the DeviceMCS device program must attend a mandatory orientation meeting (time and location to be announced). Prior to obtaining  As a condition of being issued a device, students must… ● Pay all participants must pay an annual technology user fee of $50.00 40.00 by cash, check or money order made payable to Deer Creek-Mackinaw CUSD 701 (unless the technology fee has been waived or reduced by Deer Creek-Mackinaw CUSD 701 for economic hardship.) ● Sign the Technology Acceptable Use Policy ● Sign the Device Agreement (this document) Xxxxxx County Schools.  As a condition of being issued a mobile device, all participants must take and pass, with a perfect score, a quiz on Chromebook care and use. Issuance & Return of Device and other equipment ● After the annual technology fee has been paid in full (or waived or reduced for economic hardship) the Device and adapter Mobile device  Devices will be issued to a student only after: o Attending mandatory orientation meeting, o Paying annual technology fee o Passing device quiz o Signing the student. ● For freshman, sophomores AUP and juniors, Devices and adapters must be turned in by returning  Student’s right to device terminates on the last day of attendanceschool year, unless terminated earlier by the MCS. The specific dateDevices will be collected on or before the last day of school year. Late fees may also be assessed if device is not returned immediately after being recalled by MCS before the end of school year. Although devices are to be returned immediately upon demand by MCS, a grace period of one week may be allowed. o Failure to return device by last day of school year or upon demand, results in a late fee of $5 per day for first 30 days, and procedures $10 per day for when next 30 days, until the device is safely returned. o Device is not returned within 60 days from the last day of school year, student and where parent/guardian may be assessed full replacement cost of device, and MCS may institute legal actions against the student and/or parent/guardian. Care, Maintenance & Inspections  Student and parent/guardian acknowledge they have received a copy of MCS Device Care and Use Guidelines and will comply at all times with specifications in document, as well as additional rules or guidelines regarding care and maintenance device.  Under no circumstances will the student install or permit to turn be installed on device any hardware, software, drivers, or other programs or devices without the units in advance written approval from the principal.  Under no circumstances will be communicated the student delete, uninstall, or attempt to circumvent any hardware, software, drivers, filters, or other programs or devices installed on the school website. ● For seniors, the Devices and adapters must be turned in device by the specified return dateMCSS.  Device may be inspected at any time by MCS officials, which is typically during with or without prior notice, either in person or remotely via the last week of classes for seniors. The specific date, and procedures for when and where to turn the units in will be communicated on the school website. ● Ifinternet or network connections, for any reason, a student needs purposes of maintenance and/or to monitor the student’s use of device (including any email communications and internet activities) to determine whether the Device beyond student is complying with the check-in date, they must request an extension from the technology director. ● When the Device terms and adapter are returned, they will be checked for damages, and fees assessed (if necessary) based on the schedule listed in the “Fees” section conditions set forth or described in this document. ● Failure to turn in any equipment by the specified due date will result in late fees, as described in the “Fees” section in this document. ● Students MUST turn in the Device  Student and adapter parent/guardian acknowledge that they received have no reasonable expectation of privacy to any data or information of any kind contained on the device, which shall at all times remain the start property MCS and which is intended to be used only for school purposes.  Student and parent/guardian further acknowledge that if any such inspection reveals evidence that the student has violated the Code of Student Conduct or any criminal law, such evidence may be used in support of a disciplinary action against the year. Items will be treated as “missing” if they are not the number that was issued. ● Alternative items not purchased by Deer Creek-Mackinaw CUSD #701 to replace lost or stolen equipment (such as a replacement power adapter) will not be accepted, and students will be charged for the missing equipmentstudent and/or shared with law enforcement.

Prior to obtaining a device, students must… ● Pay an annual technology fee of $50.00 by check or money order made payable to Deer Creek-Mackinaw CUSD 701 (unless the technology fee has been waived or reduced by Deer Creek-Mackinaw CUSD 701 for economic hardshipTechnology.) ● Sign the Technology Acceptable Use Policy ● Sign the Device Agreement (this document) Issuance of Device and other equipment ● After the annual technology fee has been paid in full (or waived or reduced for economic hardship) the Device and adapter will be issued to the student. ● For freshman, sophomores and juniors, Devices and adapters must be turned in by the last day of attendance. The specific date, and procedures for when and where to turn the units in will be communicated on the school website. ● For seniors, the Devices and adapters must be turned in by the specified return date, which is typically during the last week of classes for seniors. The specific date, and procedures for when and where to turn the units in will be communicated on the school website. ● If, for any reason, a student needs use of the Device beyond the check-in date, they must request an extension from the technology director. ● When the Device and adapter are returned, they will be checked for damages, and fees assessed (if necessary) based on the schedule listed in the “Fees” section in this document. ● Failure to turn in any equipment by the specified due date will result in late fees, as described in the “Fees” section in this document. ● Students MUST turn in the Device and adapter that they received at the start of the year. Items will be treated as “missing” if they are not the number that was issued. ● Alternative items not purchased by Deer Creek-Mackinaw CUSD #701 to replace lost or stolen equipment (such as a replacement power adapter) will not be accepted, and students will be charged for the missing equipment.
