Failure to Demonstrate Improvement in Areas of Deficiencies. 7.16.6.1 If the Department determines that Contractor has not made significant progress in monitoring or carrying out its QAP, including quality improvement plan or demonstrating improvement in areas of deficiencies, as identified in its HEDIS® results, quality monitoring, or Performance Improvement Project, the Department will provide notice to Contractor that Contractor shall be required to develop a formal Corrective Action Plan (CAP) to remedy the breach of Contract. The CAP must be submitted with the signature of Contractor’s Chief Executive Officer and is subject to approval by the Department. The CAP must include, but is not be limited to, the following:
