The Faculty Evaluation Forms clause establishes the requirement and process for assessing faculty performance using standardized forms. Faculty Evaluation Forms. The College will be responsible for developing forms for use in evaluations and observations, including administrator, student, peer, self, librarian, counselor, and on-line instruction evaluation forms. Additionally, the College will be responsible for creating the Service Work Plan and Professional Development form(s) as described in this Article. These forms will be developed by the College and approved by the Federation. During spring quarter of each academic year, the evaluation process may be reviewed at the request of the College or the Federation.
