Evaluator Comments. Evaluator's comments may include recommendations resulting from observations and conferences with the evaluatee. Recommendations shall be specific and shall provide direction to the evaluatee for improvement. An evaluatee shall be required to share with the evaluator data pertinent to a complete evaluation. This may include such sources as grade sheets, student projects, and student surveys. The evaluator shall indicate satisfactory or unsatisfactory for each data source. Recommendations for improving deficiencies must be included.

This performance evaluation report and any Employee comments will be filed in the Employee's personnel folder.
