Employee's Obligation for Notification of Changes of Status for Coverage. Employees must inform the Employer and the Administrator of any change in status which affects the employees' coverages. This includes change in marital status and addition or deletion of dependents. The Employer and the Union’s Health and Welfare Plan are absolved of any and all responsibility in relation to an employee's failure to provide change of status information.
