The FSMC shall submit the following supporting documents to the LEA monthly: • Daily meal counts by eligibility category by school site for each program in order to meet claim deadlines. • Daily menu production records by program. • Monthly summary of value of USDA donated foods received. • Monthly summary of revenues from other sales. • Monthly operating statements to the LEA by the fifteenth (15th) of the next month. This statement shall reflect all activity for the previous calendar month. • Invoices that show the FSMC has credited the LEA for the value of all USDA donated foods received for use in the LEA’s food service in the school year, including the value of USDA donated foods contained in processed end products. [7 CFR 250.54] • Evidence of the FSMC’s procurement of processed end products on behalf of the LEA.

If a Single Audit is required, the Subrecipient will submit a copy of the audit report to the granting Party within 9 months. If a single audit is not required, only the Subrecipient Annual Report isrequired. For fiscal years ending before December 25, 2015, a Single Audit is required if the subrecipient expends $500,000 or more in federal assistance during its fiscal year and must be conducted in accordance with OMB Circular A-133.   • Form of Agreement and Reporting If a vendor submitting an offer requires TIPS and/or TIPS Member to sign an additional agreement, a copy of the proposed agreement must be included with the proposal to the TIPS Member. TIPS does not require a review a TIPS Member’s Job Order contract TYPE AIA or other similar Contract provided by the TIPS Member. This clause does not relieve the Vendor from the responsibility to report the contract execution and the amount of the contract and any change orders.
