Dental Program. The State will provide a dental plan for the employees and their family. The coverage shall be $1,200 under the dental program to be effective upon the expiration of the current dental program. Effective January 1, 2009, the following dental change shall be in effect: Dental plan crown coverage shall be changed to 80%.
Dental Program. Effective July 1, 2009 dental insurance shall be provided by the City for all employees at no premium cost, not to exceed $118.00 per month. Spouses and dependents will be covered if economically feasible under the financial provisions of this Section.
Dental Program a. Plymouth-Canton Community SchoolsBoard of Education shall pay the cost of dental coverage up to full family coverage. See Appendix H for details. b. Dental coverage will begin on the first day of the month following employment.
Dental Program. The City shall provide a dental plan and pay the total premium costs for the employee and eligible dependents for the term of the Memorandum of Understanding. The annual maximum benefit amount is two thousand dollars ($2,000) per person. Orthodontic coverage shall be provided for dependent children under the age of twenty-six (26) years and is 50% of the dentist’s allowed fee (subject to a $1,000 lifetime maximum per dependent child).

In the event that the actual premium costs exceed the set premium rate, the Employer will provide notice to the Union, and any shortfall will be borne immediately by the Residents enrolled in the Dental Plan until such time as the terms of the Plan may be adjusted. In the event the actual premium cost for the Dental Plan is less than the set premium rate at the time of implementation, the resulting surplus will accumulate and be used to absorb future actual premium costs that may exceed the set premium rate at time of implementation.
Dental Program. 233 Dental Program - The dental program will be provided to all regular full-time 1585 members. Fifty percent (50%) or greater regular part-time employees may enroll themselves and their eligible dependents with a proportionate University contribution (based on the number of dependents covered.) a. Fees for the below mentioned services will be covered on a usual, customary, and reasonable basis with a fifty percent (50%) co-payment. b. Basic diagnostic, preventative, emergency palliative, restorative, oral surgery, endodontics, periodontics, and prosthodontics subject to a $600 annual maximum per individual. c. Orthodontics subject to a $600 lifetime maximum per individual. d. The dental program benefits will be provided consistent with the carrier's conditions and procedures. -234 Regular employees will have the option to select a managed care dental plan. The program will be provided by a carrier selected by MSU. These benefits will be provided consistent with the carrier?s conditions and procedures. Dental Program. The parties agree that the Board contribution to the employee dental plan shall be increased as follows: The Board contribution to the dental plan shall be increased 2.25% for the 2005-06 school year. The Board contribution to the dental plan shall be increased 0% for the 2006-07 and 2007-08 school year based on a freeze or decrease in overall dental premium for the 2005-06 and 2006-07 school year; otherwise the Board contribution shall be 2.25% for the 2006-07 and 2007-08 school year.
Dental Program. A dental program shall be provided by the Board for all employees in the bargaining unit. The dependents of bargaining unit members may be covered for primary or secondary coverage. Both parties agree that the Board has the right to require employees seeking primary dependent dental coverage to attest that their dependents are not receiving primary coverage from another source. A. For individual coverage, the Board will pay the 85% of the monthly premium for full-time employees and pay a proportional amount for less than full-time employees. B. For full-time employees who qualify for primary dependent (family) coverage, the Board will pay $10 of the additional monthly premium, and the Board will pay a proportional amount for less than full-time employees. When all dependents are receiving secondary coverage, the entire monthly premium of the dependent coverage shall be paid by the employee.
Dental Program. Dental Program shall be increased to Program #3 effective July 1, 1987, and shall continue for each year thereafter. Program #3 for New Jersey Dental Service Plan shall be defined as 90/10 coverage. In addition, the ortho portion for said coverage shall be increased to $1,250. The Board shall continue to pay full premium cost for employee and family.
Dental Program. Effective for work performed on and after October 1, 1964, the Employer shall contribute to a Dental Plan the sum of five cents (5c) for each hour worked or paid for, exclusive of overtime hours, for each Employee em­ ployed under this Agreement. The details of such Plan shall be completed by March 1, 1965, by a subcommittee on which the Employer and the Union shall be equally represented and shall comply with all pertinent legal requirements.
