Contractor shall certify in writing on an annual basis that it has credentialed staff that comply with DHCS requirements including, but not limited to, the following elements of this Contract: 1) Exhibit D.26.: Conflict of Interest 2) Exhibit E.6.: Screening for i. Inspector Generals’ Excluded Provider List and Medi-Cal List of Excluded Providers ii. Social Security Death Master File
