Claim Documentation. The Policyholder or Insured Member (or Nominee or legal heir if the Primary Insured Member is deceased) shall (at his expense) give the documentation specified below and any additional information or documentation specified in the Benefit provision under which the Claim is being made to the Company immediately and in any event within 30 days of the occurrence of the Injury. a) The following information and documentation shall be submitted to the company in accordance with the procedures and within the timeframes specified in Clause 4 of the Policy in respect of all Claims: i. ▇▇▇▇ completed and signed Claim form, in original; ii.
