Common use of Care Plan Clause in Contracts

Care Plan. EIS Coordinator shall create a care plan for each Client which shall include: (i) goals and objectives specific to the process of linking Client to care; (ii) identifying a responsible party for each goal and objective; (iii) regular monitoring and assessment of Client progress; and (iv) the signature of the individual providing the Service and/or the supervisor, as applicable (collectively referred to as the “Linkage Plan”).
