Common use of Care Plan Clause in Contracts

Care Plan. 40:01 The parties agree to the continuation of the Vision Care Plan with the following changes: (a) The basis for payment for covered services shall be the 2009 Optometric or Ophthalmological Fee Guide; (b) The 2012 and 2013 Fee Guides will be implemented effective April 1 of each respective year; (c) Changes to the Dental plan respecting Maternity Leave and pro-rated family coverage for part-time employees will also apply to the Vision Care Plan; (d) Part-time employees are eligible for family coverage based on sixty percent (60%) of the annual maximum per claimant.
