The Address Confirmation clause requires parties to verify and confirm their current contact or business addresses as part of the agreement. Address Confirmation. The last known address of persons in the Settlement Class will be subject to confirmation or updating as follows: (a) the Claims Administrator will check each address against the United States Post Office National Change of Address Database before the initial mailing; (b) the Claims Administrator will conduct a reasonable search to locate an updated address for any person in the Settlement Class whose Settlement Notice is returned as undeliverable; (c) the Claims Administrator will update addresses based on any forwarding information received from the United States Post Office; and,

We require either an original or certified copy of any one of the following documents which must clearly state the individual's name and permanent residential address and be dated within the last three months: • a bank, credit card or mortgage statement; • a utility bill; or • a local authority, council tax or utility bill. For any account holder under the age of 18 who may not have any of the above photographic identity documents: • Certified copy of birth certificate.
