The Depositor hereby consents to the servicing of the securities account as per the Depositary Institution’s Fees that apply to calculating the fee (please select as appropriate):

Керуючись статтею 634 Цивільного кодексу України, шляхом підписання цієї Заяви про приєднання до Договору про обслуговування рахунку в цінних паперах (надалі – Заява про приєднання) Депонент приєднується до Договору про обслуговування рахунку в цінних паперах, розміщеного на офіційному веб-сайті депозитарної установи - Товариства з обмеженою відповідальністю «ІНВЕСТИЦІЙНИЙ КАПІТАЛ УКРАЇНА» (надалі

– Депозитарна установа), xxx.xxxxxxxxxxxxx.xxx.xx .

Pursuant to Article 634 of the Civil Code of Ukraine, by executing this Deed of Accession to the Securities Account Service Agreement (the Deed of Accession), the Depositor accedes to the Securities Account Service Agreement as published on the official website of the depositary institution INVESTMENT CAPITAL UKRAINE, LIMITED LIABILITY COMPANY (the Depositary Institution), xxx.xxxxxxxxxxxxx.xxx.xx .

1. Депонент, підписанням цієї Заяви про приєднання беззаперечно підтверджує, що він ознайомився з Договором про обслуговування рахунку в цінних паперах з усіма Додатками до нього (надалі – Договір), з внутрішніми документами Депозитарної установи, Тарифами, розміщеними на офіційному веб-сайті Депозитарної установи xxx.xxxxxxxxxxxxx.xxx.xx, а також розуміє порядок внесення зміни до відповідних документів, надання послуг електронними засобами, що установлені Договором, та повністю погоджується з ними та зобов’язується їх виконувати.

1. The Depositor by signing of this Deed of Accession implicitly acknowledges that it/he/she has read the Securities Account Service Agreement with all and any Annexes thereto (the Agreement), the Depositary Institution’s internal regulations, and the Fees as published on the Depositary Institution’s official website at xxx.xxxxxxxxxxxxx.xxx.xx and understands the procedure for amending the above documents, providing services by electronic means, as set forth in the Agreement, fully accepts the above documents, and undertakes to comply with them.

2. Депонент підтверджує, що Депозитарною установою йому надана інформація, зазначена в Законі України «Про фінансові послуги та державне регулювання ринків фінансових послуг».

3. Підписуючи цю Заяву про приєднання Депонент підтверджує, що розміщення на сайті Депозитарної установи змін до Договору, Тарифів, внутрішніх документів Депозитарної установи, є належним виконанням останньою обов’язку щодо додержання форми та порядку повідомлення Депонента про зміни. Депонент безумовно бере на себе ризики та обов’язок самостійно відстежувати наявність відповідних повідомлень Депозитарної установи на офіційному веб-сайті Депозитарної установи xxx.xxxxxxxxxxxxx.xxx.xx.

4. Підписанням цієї Заяви про приєднання Депонент приєднується до Договору, розміщеному на веб-сайті Депозитарної установи xxx.xxxxxxxxxxxxx.xxx.xx, який вважається новою редакцією Договору про відкриття рахунку у цінних паперах/договору про обслуговування рахунка в цінних паперах № від . .20 р.

2. The Depositor hereby confirms that it has received information from the Depositary Institution that is set out in the Law of Ukraine «On Financial Services and State Regulation of Financial Services Markets».

3. By signing of this Deed of Accession the Depositor acknowledges that publishing on the Depository Institution web-site of the amendment to the Agreement, Fees, internal regulations makes the due execution by the Depository Institution of its obligation a regards meeting the form and order of notification of the Depositor on any changes. The Depositor unconditionally accepts all risks and liability to monitor availability of any respective notifications of the Depository Institution on its official web-site xxx.xxxxxxxxxxxxx.xxx.xx.

4. By executing this Deed of Accession, the Depositor accedes to the Agreement that is published on the Depositary Institution’s website at xxx.xxxxxxxxxxxxx.xxx.xx and that is considered to be a restated version of Securities Account Agreement / Securities Account Service Agreement No. , dated . . 20 .

Примітка: цей пункт застосовується щодо тих Депонентів, які уклали з Депозитарною установою договір про відкриття рахунку в цінних паперах/договір про обслуговування рахунку в цінних паперах до дати підписання цієї Заяви про приєднання.

Note: this Clause applies to those Depositors who entered into the Securities Account Agreement / Securities Account Service Agreement with the Depositary Institution before the date of this Deed of Accession.

5. Депонент підтверджує і засвідчує, що вся інформація та документи (зокрема за якими Депозитарній установі Депонентом доручається проведення депозитарних операцій за рахунком у цінних паперах Депонента), надані Депозитарній установі, в тому числі засобами електронної комунікації, системи BankID або іншої системи електронної дистанційної ідентифікації, є актуальною, повною і достовірною у всіх відношеннях. Депонент зобов’язується повідомляти Депозитарній установі про будь-які зміни в цій інформації та/або документах, не пізніше, ніж через 5 (п’ять) робочих дні від настання таких змін та бере на себе всю відповідальність за достовірність такої інформації та документів.

5. The Depositor confirms and certifies that all information and documents (including those that instructs the Depositary Institution to carry out depositary transactions in the Depositor’s securities account) provided to the Depository Institution also via the electronic means of communication, BankID system or other system of electronic distance identification, are true, actual and full in all matters, and the Depositor is obliged to notify the Depository Institution on any changes in such information and/or documents not less than within 5 (five) business days from the day thereof and assumes full liability for the accuracy of such information and documents.

6. Підписанням цієї Заяви про приєднання Депонент (фізична та/або уповноважена особа, що підписала Заяву про приєднання й зазначена в його преамбулі) як Суб’єкт персональних даних надає добровільний та однозначний дозвіл Депозитарній установі на:

6.1. обробку та використання своїх персональних даних без обмежень з метою виконання Депозитарною установою обов’язків за Договором, а також з метою ведення обліку операцій, що пов’язані з виконанням Договору відповідно до чинного законодавства України;

6.2. передачу (зокрема транскордонне розкриття) своїх персональних даних з правом їх обробки та використання третім особам (зокрема фінансовим установам), залученими Депозитарною установою на договірних відносинах у зв’язку з виконанням нею обов’язків за Договором та/або з метою повернення заборгованості перед Депозитарною установою за Договором;

6.3. здійснення Депозитарною установою прав та виконання обов’язків за іншими відносинами між Депозитарною установою та Депонентом/ Керуючим / Власником/- ами персональних даних / Іншим(и) власником/-ами персональних даних.

6. By executing this Deed, the Depositor (individual and/or authorized person, who signed this Deed od Accession and stated in the preamble) as the Personal Data Subject hereby provides the Depository Institution its voluntary and explicit consent to:

6.1. his/her personal data processing and utilization without limitation for the purposes of execution by the Depository Institution of its obligations under this Agreement as well for keeping records of transactions, related to the Agreement execution pursuant to the Ukrainian legislation;

6.2. transfer (including cross-border disclosure) of his/her personal data with the right of their processing and utilization to the third parties (including also financial institutions), contracted by the Depository Institution due to this Agreement execution and/or with the purpose of recovery of the indebtedness towards the Depository Institution under this Agreement;

6.3. exercise by the Depositary Institution of its rights and performance of its obligations arising out of other relations between the Depositary Institution and the Depositor / Personal Data Controller(s) / Other Personal Data Controller(s).

7. Підписанням цієї Заяви про приєднання Суб’єкт персональних даних запевняє в отриманні від Депозитарної установи повідомлення про власника персональних даних, склад та зміст зібраних персональних даних, права суб'єкта персональних даних, передбачені нормами законодавства щодо захисту персональних даних, мету збору персональних даних та осіб, яким передаються персональні дані. Власник персональних даних надає Депозитарній установі свою однозначну згоду на передачу (поширення) Депозитарною установою Персональних даних, зміну, знищення Персональних даних або обмеження доступу до них без необхідності надання Власнику персональних даних письмового повідомлення про здійснення зазначених дій.

7. By executing this Deed of Accession, the Personal Data Controller confirms that he or she has been notified of the Depository Institution notification on the Personal Data controller, composition and content of the collected Personal Data, rights of the Personal Data Subject as set forth in the Personal Data Protection legislation, purpose of collecting his/her Personal Data, and persons to whom his/her Personal Data are disclosed. Personal Data Controller grants the Depositary Institution his or her unambiguous consent to disclosure, including cross-border disclosure, by the Depositary Institution of the Personal Data to the Third Parties, and modification, destruction of, or restriction of access to, the Personal Data without the need to give written notice of the above actions to the Personal Data Controller.

8. Підписанням цієї Заяви про приєднання Депонент підтверджує наявність згоди фізичних осіб, чиї персональні дані передаються/можуть передаватися Депозитарній установі від імені Xxxxxxxxx та/або для надання послуг Депоненту (інші власники персональних даних), на передачу Депозитарній установі та обробку Депозитарною установою персональних даних цих осіб із метою, зазначеною в підпунктах 6-7 цієї Заяви про приєднання, а також факт ознайомлення цих осіб з їхніми правами, передбаченими Законом, метою обробки Депозитарною установою персональних даних, інформацією щодо осіб, яким передаються персональні дані.

8. By executing this Deed of Accession, the Depositor acknowledges that individuals whose Personal Data are / may be disclosed to the Depositary Institution on behalf of the Depositor and/or for the purpose of rendering services to the Depositor (the “Other Personal Data Controllers”) have given their consent to the disclosure to, and processing by, the Depositary Institution of the Personal Data of those individuals for the purpose set out in Clauses 6 to 7 of this Agreement and that those individuals have been advised of their rights conferred on them by the Law, the purpose for which the Personal Data are processed by the Depositary Institution, and information with respect to the persons to whom the Personal Data are disclosed.

9. Власноручний підпис, печатка (за наявності) наведені у картці зразків підписів Розпорядника(-ів) Рахунку, або їх аналоги (у тому числі, його електронний підпис, кваліфікований електронний підпис, електронна печатка (за наявності)), є обов'язковим при здійсненні всіх операцій, які буде виконувати Депозитарна установа на підставі Розпоряджень, наданих Депонентом як у письмовій так і в електронній формі.

Депонент визнає, розуміє та усвідомлює, що електронний/кваліфікований електронний підпис є аналогом власноручного підпису та його накладення має рівнозначні юридичні наслідки із власноручним підписом на паперових документах. Депонент підтверджує, що всі наступні правочини (у тому числі підписання Розпоряджень, договорів, угод, звітів, актів, листів, виписок, повідомлень тощо) можуть вчинятися Розпорядником Xxxxxxx та/або Депозитарною установою з використанням електронного/кваліфікованого електронного підпису.

9. The handwritten signature and the seal (if any) put in the certificate of authorized signatures of Account Signatory(ies) or its analogue (including their electronic signature, qualified electronic signature and electronic seal (if any)) is mandatory for performance of all transactions that the Depository Institution shall perform based on the Instructions delivered by the Depositor in written or electronic form.

The Depositor acknowledges, understands and realizes that the electronic/qualified electronic signature is the analogue of the handwritten signature and its appliance has the equivalent legal consequences as the handwritten signature on the paper documents. The Depositor do confirm that all further transactions (including signing of Instructions, agreements, deals, reports, acts, letters, statements, notices etc.) may be executed by Account Signatory and/or the Depository Institution by appliance of electronic/ qualified electronic signature.

10. З дати приєднання до Договору шляхом підписання цієї Заяви всі попередні домовленості, угоди та/або договори щодо депозитарного обслуговування, укладені Депонентом з Депозитарною установою, втрачають чинність та Договір вважається новою

єдиною редакцією всіх та будь-яких таких укладених до цієї дати договорів

10. From the date of accession to the Agreement by executing this Deed, all the previous arrangements, contracts and/or agreements as to the depository services entered into by and between the Depositor and the Depository Institution, ceases to be effective and the Agreement

shall make the new and only wording of all and any thereof.
