Plan Design Document definition

Plan Design Document means the benefit document prepared by the Contractor in conjunction with the County and approved in writing by the County, which is used by the Contractor in processing prescription drug claims in connection with this Contract.
Plan Design Document means various documents or forms, including implementation forms, clinical management forms, clinical utilization or other documents, prepared by Caremark and approved by Participating Group, as may be modified by Participating Group from time to time in accordance with Section 6.3 of this Agreement, which detailthe relevant parts of the Plan for prescription drug benefits and clinical programs adopted by Participating Group and which are used by Caremark to provide Services under this Agreement.
Plan Design Document or “PDD” means the document prepared by Caremark and approved in writing by Participating Group that may be amended by Participating Group, from time to time, in accordance with this Agreement, which documents the relevant parts of Participating Group’s Plan for prescription drug benefits and which is used by Caremark to process Claims under this Agreement.

Examples of Plan Design Document in a sentence

  • Client will provide a Plan Design Document for the EGWP plan administered by Administrator in sufficient detail to permit Administrator to perform its duties and obligations under this EGWP Addendum.

  • Participating Group will provide a Plan Design Document for each plan administered by PBM in sufficient detail to permit PBM to perform its duties and obligations under this Agreement.

  • Client shall communicate annually with Participating Group about timing of changes to Plan Design Document.

  • Prior to providing an Eligible Member any of the Covered Prescription Drug Services to which the Eligible Member is entitled under the Plan Design Document, PBM will require the Participating Pharmacy to collect any Cost Sharing from the Eligible Member.

  • PBM shall implement Plan Design Document changes and/or updates within five (5) business days of receiving complete documentation when system coding changes are not required.

  • Any changes to the Plan Design Document will be submitted by Participating Group to PBM through a revised Plan Design Document no less than thirty (30) days prior to their intended implementation by Participating Group to permit timely implementation and minimal disruption of Claims Processing Services for those Eligible Members.

Related to Plan Design Document

  • design document means any record of a design, whether in the form of a drawing, a written description, a photograph, data stored in a computer or otherwise.

  • Design Documents means, collectively and as applicable, the Conceptual Design Documents, Schematic Design Documents, the Design Development Documents and the Construction Documents prepared by the Trade Contractor.

  • layout plan means a plan of the project depicting the division or proposed division of land into plots, roads, open spaces, amenities, etc. and other details as may be necessary;

  • Plan Document means the retirement income fund agreement or declaration of trust that governs the RRIF;

  • Plan Documents means the reports, proposals, preliminary plans, survey and basis of design data, general and detail construction plans, profiles, specifications and all other information pertaining to equipment.
