Miscellaneous Record definition

Miscellaneous Record means any document, correspondence, form, report, message or recorded information regardless of physical form or characteristics created, sent organized or received by the Case Manager in the course of Case Management services provided to the Client, including but not limited to paper documents, email, drawings, charts, video tapes, photographs, phone records, data compilations, planners, calendars, dairies, draft documents, electronically stored information and metadata. DSHS Forms 11-064 and 11-065, intake summary and case notes are not Miscellaneous Records.
Miscellaneous Record means any document, correspondence, form, report, message or recorded information regardless of physical form or characteristics created, sent organized or received by the Family Mentors in the course of Contracted services provided to the families, including but not limited to paper documents, email, drawings, charts, video tapes, photographs, phone records, data compilations, planners, calendars, dairies, draft documents, electronically stored information and metadata. DSHS Forms, , , and case notes are not Miscellaneous Records.
