Medical Chart definition

Medical Chart means any clinical and/or claims data or information for a Blue Cross NC MA Member who receives or has received services from Provider that is needed by Blue Cross NC to satisfy its reporting obligations to the National Committee for Quality Assurance (“NCQA”), CMS, or any other accreditation or regulatory entity. The termMember Data” shall be inclusive of all data regardless of whether such data is stored by Provider in an electronic format or in paper records. For the sake of clarity and to avoid confusion, Member Data shall not be limited to data on only those Blue Cross NC Members who are Blue Cross MA Members but shall include data any Blue Cross NC Member who received services from Provider.
