Artificially assisted technology, such as, but not limited to, artificial insemination (AI) and intrauterine insemination (IUI).

Aetna will apply and administer its clinical infertility policies in a manner which allows for treating physician peer to peer input regarding whether any IUI cycles needed to qualify for IVF coverage, i.e., those pursuant to Aetna’s “lesser before greater” policy, need to be medicated versus unmedicated.
For self- funded plans that Aetna administers that include coverage for infertility services, Aetna will incorporate the New IUI Policy in the same manner as its fully insured commercial plans, subject to a plan sponsor's request for a deviation.
This pre-payment will typically cover 2 cycles of ultrasound, trigger shot and IUI.
