  • The employee shall give notice to the appointing authority of the need for FML by completing the required forms.

  • If an employee does not submit a medical certification, FML may not be granted.

  • Following FML leave, the employee is entitled to return to the same or an equivalent job upon return from leave.

  • Such paid leave usage will be counted against the employee's FML duration entitlement.
